Great book, I assume. Way more than I need as a patient, but does help keep things organized.

this is a soft-sided spiral notebook with heavy duty waterproof covers and many generously-sized perforated 2 page spreads for HPI note taking, followed by a fold out one-stop shop for medical references. all pages have a good weight to them and a subtle gloss, with the reference spread as a heavier stock print out which could also be easily torn out and pinned somewhere for easy reference. the only caveat to this notebook, as likely obvious to the prospective buyer, is that the notebook itself won't nicely fit in a scrubs pocket, but given the ready perforated pages, one could potentially either hold it when heading to the patient or tear out the corresponding pages for good reference with a folding clipboard that in turn can be dropped into a scrubs pocket. i've reviewed one, in case you're wondering. super handy dandy and great for a newbie to the wards to get things sorted and understood with style.

You can use these at different levels, but I found them particularly helpful as guides for my junior nursing students as they began to see "the big picture" of history and assessment. The spiral construction makes it easy to flip pages. The baseline norms and other data is a good learning tool and a visual reinforcement for new learners. Well made, printing all very clear and crisp.

Very convenient to use and simple to understand reference material for nurses and student nurses. Although facilities use EMR this notebook allows for quick jotting down of patient information. Paper is reasonably thick and takes gel ink well. Only thing I would change is adding a heavy duty back for easier writing on the go.

This book is amazing. Get it for the nursing/medical/PA student in your life! The spiral bound book is well-made. The paper is surprisingly thick. Great purchase-- would highly recommend for a gift.

Learning how to do a history and physical is essential for medical students and nursing students as well. When I working in the emergency department, the nurse would see the patient first and do a quick H&P to be able to judge the immediate needs of the patient. Although an electronic medical record is now used in most facilities, access to the computer may not be a given so taking notes on a patient is very important so that by the time you are able to stop and electronically enter the information you haven't forgotten. The templates are logically formatted. The back refence card has multiple assessment factors so that you can determine acid base abnormalities, auscultation of heart sounds, guidelines for sepsis etc. Lab values are also covered. A great guide would also be an excellent gift.

This is a great little book for H&P, labs, and quick reference. The pages can be torn out easily. It’s too big to fit into my scrub or white coat pockets, but as long as you’re not expecting a pocket sized book, it’s great. It’s about the size as an average notebook planner or calendar. The larger size makes writing spaces more effective, but a bit harder to carry handsfree. Overall I’m really happy with it.
