Do you have to lift mattress every time sheets are changed? Or just the first time they are installed?

Only the first time! I tried another reviewer suggestion - when I unhook from old sheet clip to the mattress to keep the strap in place until you have new sheet ready. Works!

Should i get one set for sheets and one for.mattress pad?

I use one set just to hold down my mattress pad, since my sheets usually stay put. You could use another set for your sheets if you have issues with them slipping off.

Does 4 piece set have 16 clips?

Yes it has 8 clips on each end. Don't pay attention to the comment above. I'm not sure where they're getting it only had 8 because you can count the clips in the picture and it clearly shows 16 clips!
