It is one "item" It holds 4 cards on each side, for a total of 8 cards. If they are paper business cards, you can often double a couple cards to make the total 9 or 10.

Will it hold 3 x 5 cards?

No - what will fit into it are business cards and things like your credit or debit cards but that's it - not big enough for 3 x 5 cards.

What are the sizes between holes? Is it 15mm and the middle gap 65mm?

The real question is from where are you measuring. The industry norm is to measure from C To C the center of pne hole to the center of the other. If you do that it is 20mm between the holes and the gap is 70mm.Could be that is what you have but you are not measuring from C TO Csam
