Will this fit over a heavy duty extension cord?

Yes. I have a thick yellow cord from a heavy duty outlet strip going though it.

How do you hold the cover down to hard floor?

Many thanks for your interest in D-Line Floor Cord Cover, they are made from a flexible material which simply uncoils to lay flat (with no need for adhesive tapes). Lo compre por que debido a la pandemia y a las fallas de internet, decidí poner un cable ethernet y para no tener peligro de tropezarme esta fue un excelente remedio. Se ve fuerte, de uso rudo y pesado, en mi caso no decidí pegarlo al suelo, solo con el peso del producto, y me ha funcionado muy bien. Al inicio cuando abri el paquete ví que venía enrollado y pensé que sería un problema, pero al momento que lo puse en el suelo se acopló muy bien. Muy recomendable
