Q: Does it make a fan work for only 15 seconds in 30 minutes interval "forever"?

I’m not sure you can set it to turn on or off for less than 1 minute at a time but, yes, you can set the interval to run indefinitely.

Q: We want it to run 5 min on/15 min off indefinitely but it seems to quit the cycle after 12 hours. Is there something we are missing in the set up?

Yes. Go back through setting timing cycle and then press confirm. It allows you to set for a certain number of hours which is on in your case but to do that you hit set instead confirm and enter the amount of hours.

Q: Does this cover both outlet?

I assume you are talking about an outlet with two jacks spaced one above the other. If that is the case and you plug the timer into the top jack, the bottom jack will still be available for another plug. However, if you plug the timer into the bottom jack, the timer will cover up the top Jack as well.

Q: does this have a battery backup in case of a power outtage?

No battery backup but you could buy a ac/dc mini generator that if the power does go out hook up your timer to the generator
