is this a small (5oz) shaker or a larger 5 oz in weight shaker?

It is the 5oz in weight shaker. I buy and use a lot of the Red Star Nutritional Yeast and love it. Personally I wish the jars was larger but they aren't that small either. They are almost as tall as a 5 pound bag of flour. This Stuff worked great for making vegan Butter, Cheesy Sauce, and a huge number of other dishes. I will definitely buy more.

Does this contain folate? Folic acid?

Contains folate. 1 serving 250% daily value

What kind of B12 is added?

The "type" of B12 is that made by yeast.

If I were to purchase a 6 pack of these, what is the expiration date?

The expiration date is going to vary but you should have at least a year. Keep it in a cool dark place and it will keep for that length of time.
