I read that these can fit a standard pillow case, however it says is a King size... why?

Is it going to be really firm that it wont make a dent? I want a medium / firm.

No. It is not firm. I consider it as medium / firm.

A note with my order said I could extend my warranty at their website. I see no option for that at the whiteclassic.com website.

Me once again after almost hour trying to find website , I went into my orders on Amazon for the pillows and saw a sign for support; click on it and it send you to White Classic website, click on warranty and register. Be sure you have your order number (found at the top of your order on Amazon). Good luck.

Will these work to support my arms while I ice my elbow?

If you're looking to raise your elbow more than a couple of inches, then no, but if you don't need a high elavation and you are looking for a soft pillow, then yes. These pillows should do nicely.
