Does it read the data from the magnetic strip on the back of the insurance card?

No, it is just an image scanner. It works very well.

Does this import the data into forms (excel or some kind of software) so the data can be manipulated or does it only scan and save an image?

I believe it just scans and outputs an image, no OCR software was included in my purchase.

Does it scan 2 sides of the card?

Not at the same time. You have to feed the card through twice, scanning front and back separately.
