What size eyelets do I need to buy to use with this?

It comes with 20 - 3/16 eyelets. This is indicated in the product description. I have made a few tags and the punch works perfectly! I am very impressed with this punch and very happy to have it! 5 Stars from me!

how big exactly is the actual tag?

The tag is rectangular 1.5" wide x 2 1/4" high. The actual rectangle size would be 1.5" x 2" but the area to punch the hole is a bit extra

Can you see where you are cutting? Is there a window or a clear window to see through?

Not a window. I usually flip it over to make sure I'm making a good cut, since I make multiple cuts per sheet of cardstock. Once you get the hang of how it cuts, it's pretty easy to gauge placement.

Can this ben used on cotton fabric?

It can be used on standard paper and thin cardboard, it is not recommended for fabric. Thank you for your great question.
