How long will the water stay hot inside the unit when not used?

If the outside temperature drops, the water temperature will gradually decrease too

Can you convert/replace the shower head with a dual head, one as is and one that is a handheld wand?

The head screws on just like the head in our indoor bathroom, so I would say yes.

Question 3

Where is the solar panel? Does the entire unit need to be exposed to the sun? We were planning to have this in a semi-enclosed outdoor shower stall.

The entire unit needs to be exposed to the sun. 
This was a gift for my husband and while he was initially thrilled, he was really annoyed that even though the details claim it fits most hoses it DOES NOT. 
He now has it rigged with layers and layers of duct tape...totally annoying, but we also read reviews about how difficult the returns were.
Though the unit works well at heating in the sun, we live in Florida,  the grey tape situation makes me wish I'd just hired a plumber to install a real outdoor shower. 
We use it in our garden area for my husband and boys to get cleaned off from working and playing outside.

Question 4

I want to attach a decorative 4” piece to the shower post. Can the variable temp handle be extended 4” out from the post?

Not sure I understand  your question. If you are asking about the shower head, it would be simple matter to extend the shower head 4" or more inches. If you are asking about the on/off hot/cold control, then no, it would be a simple matter, if doable at all, to extend that control. You would have to detach the control from the tower and then take it to  plumbing store and see if you can match it to what you want to use.
