I have done both day and night and I would say that if you wear it during the day you cannot do much movements as the velcro part that is wrapped around your arm (on the lower part) keeps falling off. But I have used it to do some rowing or activities like that so that's probably why... When it comes to the night it is up to you but I would advise you to leave it at a comfortable tightness... I explain, I have used it many times at night and it really helped my elbow to rest and I felt much better in the morning BUT last time I used it I was in pain in the morning. I don't think it was too tight but it might have been the case or I put my arm in an unpleasant position during the night and woke up with a different kind of pain that was as unpleasant as the one I had fallen asleep with. But this is my experience... It all depends on the kind of injury you have and so many other things I guess... I just hid half of it higher up my arm and it looked ok. I found it very comfortable to wear but you have to be very careful not to overtighten as one day i had tingling all day in my hand while wearing it . I had not relized i had the strapping too tight. The velcro patches caught my hairs on my arm but it just took time to get the right band position. You have a lower and upper arm strap and you can adjust the pressure to suit.
