You wear them uber your clothes to keep you warm. My husband says they are awesome and keeps him warm. **Q: What size should I order if my boyfriend is a large shirt but usually a small/med in underwear** A: Just my opinion, I would get a Medium-Large. Pajamas are usually big for their stated size; however, you want more of a tight fit with thermal underwear as to keep the heat in, and usually warn under regular clothing. So I would say medium. Everything fits differently, but I would say the medium would be a better choice under a large shirt. **Q: this is fleece lined?** A: It is not "fleece" material - however, the warmth is similar to that but is instead a form fitting base layer. I wear these under pants during New England winter temperatures and it is just as warm as smartwool. I also wear them under my motorcycle gear during this season too.
